

Overview
Xerostomia (dry mouth) is caused by reduced or altered saliva production. Saliva is critical — it neutralises acids, washes away food, and prevents infection. Without adequate saliva, tooth decay and oral infections develop rapidly. Dry mouth is common in Kuwait among the elderly, people on multiple medications, and those with diabetes.
Symptoms
- Persistent dryness or stickiness in the mouth; thick, stringy saliva
- Difficulty chewing, swallowing, or speaking; frequent thirst
- Burning or tingling sensation; cracked lips and corners of mouth
- Altered sense of taste
- Rapid widespread tooth decay — a hallmark of severe xerostomia
Causes & Risk Factors
- Medications — most common cause: antihistamines, antidepressants, antihypertensives, diuretics (commonly prescribed in Kuwait)
- Ageing; uncontrolled diabetes
- Sjogren's syndrome: autoimmune attack on salivary glands
- Radiotherapy to the head and neck
- Mouth breathing, snoring, and dehydration — relevant in Kuwait's heat
Complications
- Rapidly progressive decay affecting multiple teeth simultaneously
- Oral candidiasis (thrush)
- Difficulty eating affecting nutrition; halitosis
- Difficulty wearing dentures; significantly reduced quality of life
Diagnosis
Clinical examination assesses saliva quantity and quality. Blood tests screen for diabetes and Sjogren's antibodies. Medication review is essential to identify drug-induced xerostomia.
Treatment
- Address the underlying cause where possible (adjust medications, control diabetes)
- Saliva substitutes and oral moisturising gels
- Saliva-stimulating medications (pilocarpine) for Sjogren's or post-radiotherapy
- Sugar-free chewing gum and frequent sips of water
- Intensive fluoride protocol: daily high-fluoride toothpaste or fluoride trays to prevent decay
Prevention
- Stay well hydrated — especially important in Kuwait's heat
- Review medications with doctor if dry mouth is a side effect
- Avoid alcohol-based mouthwashes which worsen dryness
- Use a humidifier at night; treat snoring or sleep apnoea
- See dentist every 3-4 months if you have dry mouth to monitor for decay
