Overview

Bruxism is the involuntary grinding, clenching, or gnashing of teeth — most commonly during sleep (sleep bruxism). It is increasingly common in Kuwait where high stress levels and anxiety are significant contributing factors. Over time, bruxism causes severe tooth wear, cracking, jaw pain, and headaches. Many people are unaware they grind until a dentist identifies the signs or a partner reports the sound.

Symptoms

  • Worn, flattened, or chipped teeth
  • Tooth sensitivity from enamel loss
  • Jaw pain and stiffness especially on waking
  • Headaches at the temples upon waking
  • Facial muscle fatigue; clicking or popping of the jaw joint
  • Scalloped edges on the tongue or ridges on the inner cheeks

Causes & Risk Factors

  • Stress and anxiety — the primary driver; highly relevant in Kuwait
  • Sleep apnoea: strongly associated with sleep bruxism
  • Certain medications: SSRIs, stimulants
  • Caffeine and alcohol consumption; genetics
  • Personality type: competitive, hyperactive tendencies

Complications

  • Severe irreversible tooth wear requiring full-mouth rehabilitation
  • Cracked teeth leading to root canals or extractions
  • Temporomandibular joint disorder (TMD)
  • Chronic headaches and facial pain; failure of dental restorations

Diagnosis

Dentist assesses wear patterns, checks jaw muscle tenderness, and examines for cracking and enamel loss. Sleep studies may be recommended if sleep apnoea is suspected.

Treatment

  • Custom night guard (occlusal splint): primary treatment — protects teeth from grinding forces
  • Stress management: therapy, mindfulness, relaxation techniques
  • Treat sleep apnoea if present
  • Botulinum toxin injections into masseter muscles: reduces grinding force in severe cases
  • Dental rehabilitation to restore severely worn teeth

Prevention

  • Manage stress — the most important modifiable risk factor
  • Limit caffeine and alcohol especially in the evening
  • Wear night guard if prescribed; replace when worn down
  • Regular dental check-ups to monitor tooth wear
  • Seek treatment for sleep apnoea
