

Overview
Cracked tooth syndrome is an incomplete fracture of a tooth causing inconsistent pain — one of dentistry's most challenging diagnoses. Pain typically appears only when biting in a specific way and disappears immediately on release. Cracks range from minor surface lines (harmless) to deep fractures that split the tooth. Without treatment, cracks worsen over time and ultimately lead to tooth loss.
Symptoms
- Sharp pain when biting down that disappears immediately on release
- Sensitivity to cold or sweet foods
- Pain that is difficult to pinpoint, coming and going
- Spontaneous pain and heat sensitivity in advanced cracks
Causes & Risk Factors
- Tooth grinding (bruxism) — common in Kuwait due to stress
- Chewing hard foods: ice, hard sweets, nuts, bones
- Large existing fillings weakening tooth structure
- Trauma from accidents or sports injuries
- Age — teeth become more brittle over time
Complications
- Crack deepens over time, splitting the tooth completely if untreated
- Pulp infection requiring root canal treatment
- Dental abscess formation; ultimately tooth loss
Diagnosis
Diagnosed by clinical examination, transillumination (light through the tooth), biting tests using a bite stick, and selective anaesthesia. X-rays may not show the crack but rule out other causes. A night guard is often trialled as both treatment and diagnostic confirmation.
Treatment
- Dental crown: holds the tooth together and protects the crack
- Root canal treatment if the crack has reached the pulp
- Dental bonding for minor superficial cracks
- Extraction if the crack extends below the gum line
- Night guard to prevent further cracking from bruxism
Prevention
- Wear a custom night guard if you grind your teeth
- Avoid chewing ice, hard sweets, or bones
- Ask your dentist about crowning teeth with large fillings before they crack
- Wear a mouthguard during contact sports
