Overview
Periodontitis is a severe gum disease that destroys the bone and soft tissue supporting teeth. It develops from untreated gingivitis — bacterial infection deepens, forming pockets that destroy the periodontal ligament and alveolar bone. Periodontitis is irreversible but can be halted with treatment. It is the leading cause of tooth loss in Kuwaiti adults, and research firmly links it to cardiovascular disease, diabetes, and respiratory disease.
Symptoms
- Gums that bleed when brushing — persistent and worsening
- Red, swollen, or receding gums; deep gum pockets
- Persistent bad breath; pus between the teeth and gums
- Teeth that feel loose, have shifted, or appear longer as gums recede
- Note: periodontitis is often painless until advanced — regular check-ups are essential
Causes & Risk Factors
- Untreated gingivitis progressing to bone destruction
- Tobacco and shisha — strongly doubles risk and severity
- Diabetes: bidirectional relationship — each worsens the other
- Genetic predisposition: up to 50% of susceptibility is inherited
- Hormonal changes, certain medications, immunosuppression, stress
Complications
- Tooth loss — the leading dental cause of tooth loss in Kuwaiti adults
- Increased cardiovascular disease risk from systemic inflammation
- Worsened blood sugar control in diabetics
- Preterm birth and low birth weight in pregnant women with severe periodontitis
Diagnosis
Full periodontal assessment: pocket depth charting, bone level X-rays, bleeding on probing, and mobility scoring. Staging (Stage I-IV) and Grading (A-C) classify severity and risk to guide treatment.
Treatment
- Root surface debridement (deep cleaning): removes infection from below the gum line — the cornerstone of treatment
- Locally delivered antibiotics placed in deep pockets
- Periodontal surgery for advanced cases: open flap debridement, bone grafting
- Regular 3-monthly periodontal maintenance to prevent recurrence
- Quit tobacco and optimise diabetes management
Prevention
- Excellent daily oral hygiene: brush and floss every day
- Professional cleaning every 6 months (3 months if at risk)
- Quit smoking and shisha — the single most impactful lifestyle change for gum health
- Control diabetes rigorously; treat gingivitis before it progresses
