

Overview
Dental fluorosis is a developmental condition caused by excessive fluoride intake during tooth formation (before age 8). It results in enamel changes ranging from faint white spots (mild) to brown staining and pitting (severe). In Kuwait, fluorosis may occur from excessive use of fluoride toothpaste in young children who swallow it, or fluoride supplements given in addition to dietary fluoride sources.
Symptoms
- White chalky spots, streaks, or patches on the tooth surface (mild)
- Yellow-brown staining of the enamel (moderate)
- Pitting, roughness, or surface irregularities (severe)
- Affects multiple teeth symmetrically — typically permanent teeth
- Painless but causes cosmetic concern
Causes & Risk Factors
- Excessive use of fluoride toothpaste in children under 6 who swallow it
- Fluoride supplements given in areas with adequate water fluoridation
- High fluoride in natural well water or certain bottled waters
- Infant formula made with high-fluoride water
Complications
- Mild fluorosis is cosmetic only; no functional impairment
- Severe fluorosis: weakened porous enamel more susceptible to decay
- Significant psychological impact in teenagers and adults self-conscious about appearance
Diagnosis
Diagnosed clinically by the pattern and nature of enamel changes. Graded using Dean's Fluorosis Index. Must be distinguished from white spot lesions from early decay, hypocalcification, and amelogenesis imperfecta.
Treatment
- Mild: microabrasion polishes away superficial white spots
- Moderate: composite resin bonding covers discolouration with natural-looking material
- Severe: porcelain veneers or crowns restore appearance and protect weakened enamel
- Prevention counselling: the most important intervention
Prevention
- Use only a rice-grain amount of fluoride toothpaste for children under 3
- Use only a pea-sized amount for children aged 3-6
- Supervise brushing and discourage swallowing toothpaste
- Do not give fluoride supplements without dentist assessment of total fluoride intake
